A care and support plan is a document that details the things you wish to achieve as part of your care and support, that have been agreed between you and the council.

If you need help adapting, maintaining or repairing your home or need a handyperson service, Care and Repair can help.

B9 - Hospital Discharge and Continuing Health Care

This information is to help you when you come out of hospital. As soon as you arrive in hospital, doctors and nurses start to plan for your discharge. As soon as it medically appropriate for you to leave you will be discharged. If it is likely that you will need care and support after you are discharged, the hospital will contact Adult Social Care and ask for you to receive an assessment.
