Reablement

Reablement is a programme of short term support tailored to your individual needs, to help you (re)learn the skills needed for daily living and reduce the amount of care and support you need. 

Reablement is about helping people to do things for themselves rather than having things done for them.

Reablement programmes can last from as little as a few days to a maximum of six weeks.

The service is focused on helping you achieve goals that you set yourself, in conjunction with the Reablement team, to maintain your independence.

As well as the satisfaction people gain from being able to do more things for themselves, input from the reablement service is likely to lead to less need for longer term care and support and often no ongoing care is required.

How Reablement works

About reablement

Reablement can help with a range of tasks including:

  • personal care tasks such as washing, dressing and toileting
  • domestic skills such as food and drink preparation and organising and planning your activities of daily living
  • encouraging the necessary confidence to manoeuvre. This would include getting up and out of a chair, getting in and out of bed and getting on and off the toilet.

Trained reablement staff will work with you to ensure that you are supported in your own home and to enable you to safely practice those skills.

We will implement your agreed goal plan with you.  This is completed during your assessment alongside an occupational therapist.  The Reablement Support staff will alwork with you to achieve the goals set. These will be based on what is reasonable for you to expect to be able to do for yourself at the end of reablement. The goals will be recorded as part of your Goal Plan and kept in your home so that you, your family/informal carers and reablement staff can monitor your achievements and identify new goals to work towards.

The emphasis for the service will always be to encourage you to do things for yourself.

As part of Reablement you may be offered equipment to help make daily living easier and also Technology Enabled which is electronic devices that can support you to maintain your safety, indendpence and wellbeing.  You can find out more on our Technology Enabled Care and Equipment and Adaptation pages.

Access to Reablement

How to access reablement

Access to reablement services is through a discussion with a social care professional.

Reablement will be considered for all adults after a stay in hospital; ward staff will identify if you are suitable for this service. There are some circumstances, such as end-of-life care, that are not suitable for reablement.

In order to access reablement you will need to be:

  • an older person, or someone over 18 years old who has a physical or learning disability
  • you need support to stay living independently at home
  • Reablement is provided for a short period and that a longer term care and support plan will be considered at the end of Reablement

Following reablement, a Financial Assessment will be required to establish any financial contribution for packages of ongoing care.

Surveys

At the end of the period of Reablement you will be asked to complete a service user survey.  We use the results of the surveys to improve our service.
