Referral Details

Accessed through PCC led panel process after an assessment of needs. Periods of support are set, (e,g: 13 & 26 weeks) with support review dates. The support is designed to meet planned outcomes for each individual child or young person. Where appropriate reviews will consider transitions into other community based activities.
