Link Care

Link Care offers short breaks to children and young people with disabilities within the homes of specialist carers.

The aim of the scheme is to match carers with families and create extended families that are able to offer short breaks on a regular basis. Most of the children and young people that access Link Care live at home with their families and these short breaks give them the opportunity to develop new relationships in a safe and caring environment.

Referral Details

Formal requests for short breaks are made at the relevant decision making panel.  Requests are made following an assesment of the child or young person's needs.  The assessment is completed by a social worker or another professional who knows the child or young person and their family such as a community nurse or a school nurse.  The child or young person's family contribute to the assessment as well as others who are involved with the family and the child or young person's views. 

The person who completes the assessment will go to the panel to present the assessment.

If it is agreed that the Link Carer can provide this support, a referral is made.  More information can be found on the Decision Making page.
