Caring Together
Caring Together is contracted by Peterborough City Council to support carers who look after a family member, partner or friend who cannot cope without their support due to long-term illness, mental health or disability - including strokes. Age UK Cambridgeshire & Peterborough
Age UK supports older people, including stroke survivors, with services such as practical and social support including befriending, social activities and support in the home. Call 0300 6669860 or email infoandadvice@ageukcap.org.uk
Alzheimer’s Society, Peterborough
Alzheimer’s Society in Peterborough work with individuals affected by dementia following a stroke and support their carers and families. Going home from hospital - British Red Cross
The British Red Cross are contracted by Peterborough City Council to offer people, including stroke survivors, support when going home from Peterborough City Hospital. The British Red Cross will collect you from hospital and make sure you have everything needed for the first 24 to 72 hours on arrival home.
They also provide practical and emotional support, whether that is stocking the fridge, feeding the cat or a just cup of tea and chat.

Stroke Association Connect (referrals through hospital discharge teams)
Newly diagnosed stroke survivors going home from hospital can be referred directly to the Stroke Connect Service by Peterborough City Hospital’s Discharge Team. The service will check on you, ensuring you are not on your own after your stroke and will offer reassurance, assistance with immediate concerns, and connection to ongoing support. Social Prescribers
If you or a family member/partner or friend have recently had a stroke you can be referred to a Social Prescriber for a range of health and wellbeing support. Talk to your GP to find out more about social prescribing.
Peterborough City Council
If you are a stroke survivor and need help with equipment and technology Adult Social Care at Peterborough City Council can offer advice and support.
If you need more support, Adult Social Care may suggest you undertake a social care needs assessment. The assessment looks at your strengths and needs, identifying what support may help you. You can find out more on our Adult Social Care Assessment and Care and Support Planning page.
