The team support people with Autism Spectrum Disorder (ASD) who may be considered vulnerable in their communities, but do not have another diagnosis. This would mean their needs could be met by other teams - for example, physical disability, learning disability, mental health or drug and alcohol teams.
The team will support young adults from the age of 18 until a time suitable to them as an individual when they will transfer to a long term Physical Disability Care Manager or remain with the AAT. This is likely to be (but not automatically) at the end of formal education.
For young people who do not need social care before the age 18, and there is a likelihood that they may need support from adult social care, the team will start discussions and planning for adulthood at an appropriate point with the young person and their family/carer from the age of 16 as part of their Education Health and Care Plan. The team will facilitate the development of a support plan that meets a person’s assessed eligible needs, promotes independence and meets identified outcomes. Services could include signposting to voluntary agencies in the community, referrals to National Autistic Society Support Workers, Assistive Technology, support in the person's own home, supporting carers, social isolation, supported living and residential and nursing home placements if appropriate.
